1、境外XX项目*项目现场Covid-19疫情期间封闭管理制度（第三版）Edition3*项目现场自3月16日实行封闭管理以来，对阻止Covid_19疫情对项目现场的影响起到了积极的作用，随着巴基斯坦Covid_19疫情持续严重，项目所在地Punjab省和Jhang市被感染人数持续增加，原来的封闭管理制度已不适应*项目现场对Covid-19疫情的防控，现对项目现场Covid-19疫情期间封闭管理重新进行修订。SincetheimplementationofclosedmanagementsystematJhangprojectsiteonMar16th,ithascontributedsignif

2、icantlytoavoidingtheimpactofCovid-19attheprojectsite.WithregardtothecontinuousseverespreadingofCovid-19inPakistan,aswelltheincreasingcasesinPunjabprovinceandJhangcity,theoriginalclosedmanagementsystemhasbecomeinappropriatefortheprecautionofCovid-19,therefore,arevisionontheclosedmanagementsystemamidC

3、ovid-19isimplemented.1总则1. GeneralProvisions1.1 为适应*项目所在地巴基斯坦PunjabCovid-19疫情防疫需要对原*项目现场封闭管理制度进行修订；1.2 InordertocomplywiththerequirementofCovid-19precautionforJhangprojectlocatedinPakistanPunjabprovince,thesystemisrevised;1.3 封闭管理的目的：通过封闭管理制度修订和在*项目现场的执行、防范Covid-19疫情对*项目现场人员健康的伤害；1.4 Objectiveofclos

4、edmanagementsystem:Throughrevisionandexecutionoftheclosedmanagementsystem,topreventhealthdamagesonpersonnelatJhangprojectsiteamidCovid-19;1.5 封闭管理范围：*项目围墙范围内、凡是进入*项目现场的所有人员，包括但不限于PTPL、NESPAK.SPU、*、分包商、供货商、服务商和临时访客；进入的所有车辆；1.6 Scopeofclosedmanagement:WithintheboundarywallofJhangproject,allthepersonne

5、lincludingbutnotlimitedtoPTPL,NESPAK,SPU,*,subcontractors,vendors,serviceprovidersandtemporaryvisitors;allthevehiclesenteringthesite;1.7 封闭管理时间：视当地Covid-19疫情发展情况由EPC承包商确定，PTPL/NESPAK作为业主有建议权；1.8 Durationofclosedmanagement:ItshallbedeterminedbytheEPCcontractorasperthelocalCovid-19epidemicdevelopment,

6、astheemployerPTPL/NESPAKisentitledwithrighttomakerecommendations;1.9 责任单位：根据EPC合同、作为EPC承包商对现场安全负责，封闭管理的制度的制定、修订、执行和终止由*负责；1.10 Responsibleunits:AccordingtotheEPCcontract,*isresponsibleforsitesafetyastheEPCcontractor,and*isresponsiblefortheformulation,revision,implementationandterminationofthisclosed

7、managementsystem;2进入现场人员管理2. PersonnelEntryManagement2.1 审批：凡是需要进入*项目现场的人员，均需要在进入现场前提前24小时填写*项目入场申请表，申请表的格式见附件1；申请人对所填写的信息的真实行负责；2.2 Approval:AllthepersonnelenteringthesiteshallfillintheEntryandExitApplicationForm,seeAppendix1fortheformatofapplicationform;Theapplicantshallberesponsiblefortheauthenti

8、cityoftheinformationprovided;2.3 管理：根据EPC合同、EPC负责厂区围墙内的安全管理、由*HSE部门负责审核入场申请人提交的申请表，审核通过后交*现场经理审批；*有权拒绝来自Covid-19疫区的人员和与Covid-19疑似和确诊病例有接触的人员入场；2.4 Management:AsperEPCAgreement,EPCcontractorisresponsibleforthesecuritymanagementinsidetheplantboundarywall,*HSEdepartmentshallreviewtheapplicationformsubm

9、itted,theformshallbesubmittedto*sitemanagerforapprovalafterreview;*isentitledtorefusetheentryofpersonnelfromCovid-19epidemicareaandpersonnelincontactwithsuspected/confirmedCovid-19cases;2.5 人员入场：经审核同意进场的人员有效申请表交给主大门安保后进入现场、所有进入人员进入现场必须戴口罩、保持2米以上的社交距离，入场后直接进入隔离区；2.6 Personnelentry：Validapprovedperson

10、nelentryapplicationformshallbesubmittedtothemaingatesecuritybeforeentry,allthepersonnelshallwearmaskwhenenteringthesite,keepnolessthan2meterssocialdistance,anddirectlyenterthequarantinearea;2.7 临时入场人员：因工作需要临时进入现场当天离开现场的人员为临时进入人员，按2.1、2.2和2.3执行，同时提交单独的Covid_19防范措施、经审核和审批同意后方可进入；临时入场的送货司机原则上不允许离开驾驶室、如

11、确需离开的、责任单位需要临时入场人员申请单上提前安排好防护措施并指定专人负责监督落实直至临时进场人员离开现场；2.8 Temporaryentrypersonnel:Personnelwhoenterandleavethesiteonthesamedayforworktemporarilyshallberegardedastemporaryentryofpersonnel,itshallbeexecutedasper2.1,2.2and2.3,independentCovid-19precautionmethodsshallbesubmittedwithreviewandapproval;in

12、principle,cargodeliverydriversareprohibitedtoleavethedrivingcab,whennecessary,responsibleunitsshallindicateontheapplicationform,arrangeprecautionmethodsanddesignatespecialsupervisortosuperviseuntilsuchtemporarypersonnelleavethesite;2.9 隔离：进入隔离区的人员最少要求有14天隔离、隔离人员每天测量体温两次并进行记录；2.10 Quarantine:Allthepe

13、rsonnelshallexecutenolessthan14(fourteen)daysinquarantine,bodytemperaturesshallbetakentwiceperdayforpersonnelinquarantine;2.11 场人员：所有生活在现场的人员公共场所和施工工地必须戴口罩、每天至少进行一次体温测量并进行记录；2.12 Sitepersonnel:Allthepersonnelatpublicareaandconstructionareainsidetheprojectsiteshallwearmask,testbodytemperatureatminimu

14、monceperdayandmakerecord;2.13 助和检查：营地内人员连续2次体温测量高于37.3度、按应急救援预案、用救护车送至Jhang定点医院检查、如Covid-19疑似和确诊禁止再次进入营地;2.14 Rescueandinspection:Personnelinsidethecampwithcontinuoustwiceofbodytemperatureexceeding37.3shallbeapplicableforemergencyrescueplan,andshallbesenttothefixedhospitalbyambulanceforinspection,if

15、suspectedorconfirmed,reentryshallbeprohibited;2.15 为准则：营地内人员严禁聚集、社交距离保持2米以上，餐厅就餐严禁大声喧哗，排队打饭和就餐保持应有的社交距离；2.16 Codeofbehavior:Personnelinsidethecamparestrictlyprohibitedtogather,socialdistanceshallbenolessthan2meters,loudnoiseisstrictlyprohibitedwhendining,appropriatesocialdistancingshallbekeptwhileliningforfoodanddining;2.17 责：违反上述Covid-19疫情期间人员管理规定的将禁止进入*项目现场,并承担所有因违规造成的损失和后果。2.18 Punishment:AllthepersonnelviolatetheabovementionedmanagementprovisionsamidCovid-19shallbeprohibitedtoenterJhangprojectsite,andshallbearallthelossandconsequencescausedbytheviolation.
